Successful Interventions for Chronic Disease Prevention

The purpose of this assignment is to provide students the opportunity to research a chronic health issue (that is different than your team project, if applicable) and interventions that try to prevent it. Students should select from the following health issues: Cardiovascular Disease, Diabetes (Type 2), Respiratory Diseases (select one), Infectious Diseases (select one) or Cancer (select one). The goal of this assignment is for the student to become more knowledgeable in one of the chronic disease health issues and research successful interventions toward prevention. The written paper should be up to 10 pages in length, double spaced, 12 point Times New Roman font with one inch margins following the APA Style in citations and formatting. The written paper should include the following sections (adapted from Hayden, Second Edition, Chapter 11, pages 291 – 296): Define the health issue. Gather information. Current data and statistics about the health issue should be detailed. Students should only use references from 2009 to the present. In addition, data about the population most affected by the health problem should be also be completed and included in the written paper. Identify possible reasons or causes for the health problem. The student should take the information received from their research and combine it with the information learned about the target population in order to identify causes for the health problem. The question that needs to be answered is: “Why does this health problem exist in the population?” Identification of successful interventions. The student should then complete a search in the literature to identify five successful interventions in regard to the health issue and the applied theory/model constructs. A summary of the five interventions, the theory/model used, how the constructs were applied, and the results of each study should be included in the paper.
