When evaluating the health of a 76-year-old Black/African-American male with disabilities living in an urban setting. There are certain things that health clinician need to consider. The health history of the patients help the clinicians with the history of the patient while they are making decisions. The health history of the patient comprises the medical history of the patients in terms of the diseases that they have been diagnosed with before. The clinicians need to understand the history of the patient in deciding the type of medication that they will recommend to the patient.

The interview needs to be about the history of the patient health wise and the medication that they got to deal with the diseases (Roldós, Hopenhayn, Sacoto, & Bustamante, 2017).  The clinician selects the related conditions that are related to the current challenge because they might be having the same solution. The techniques used in collecting the information include the friendly technique. The clinicians pretend to be friendly to the patients so that they can release all the information they have. The potential health-related risk is the disease that re-emerges again after it has been treated.

Using the techniques based on the patient age one will know the most affected age group and gender. When the clinician has the data, they can come up with mitigating plans to deal with the challenge (Roldós, Hopenhayn, Sacoto, & Bustamante, 2017). The risk assessment tools include age and gender, and they enable the clinician to know the amount of dosage to recommend to the patients. It is applicable because some diseases affect more in particular ages and gender. The questions likely to be used by clinicians include

  1. Have you had faced a similar challenge before?
  2. How long have you been unwell?
  3. What are the signs and symptoms of various diseases that you cover?
  4. Have you taken any drugs to cure the disease without a prescription of the better?
  5. What are the challenges that you face and result lack of drugs?
